At St. Mary’s PHO, Better Care, Better Outcomes is defined by a solid framework for delivering high-quality, cost effective patient centered care. The practice infrastructure, contract negotiation, and clinical integration commonly referred to as population health management is the core of the PHO and demonstrated by:
- Providing a foundational model for physicians under a common infrastructure.
- Allowing physicians to align economic incentives for improvement in cost, quality and efficiency thru evolving payer reimbursement models in Value Based Contracts.
- A governance structure that has been developed and oversees PHO business strategy and operations; The structure allows for multiple opportunities for physicians to engage in participation.
- Focused clinical integration through adoption of peer-to-peer protocols for identified conditions across the care continuum.
- Access to PHO and physician leadership to assist in best practice engagement, performance improvement, payer navigation and issue resolution
- Seasoned staff expertise to work with physician and office staff to enhance contract performance outcomes in both quality and cost in addition to optimized compensation support (on-site support).
