Value-Based Contracting
We have alignment with all of the major payers in Michigan serving over 40,000 covered lives. We maintain comprehensive payer relationships, negotiate, evaluate, and administer the contracting process, and ensure reimbursement is structured on value-based principles.
Financial Technology Support & Management
Implementation of the technical infrastructure needed to support Health Information Exchange (HIE) initiatives through multiple payer opportunities. Continued management with third party interface maintenance. Primary goal: physician access to patient information at point of care.
Health Focus-Population Health Patient Registry
PHO funded and sponsored patient registry (Health Focus) which includes a Patient Risk Stratification Tool, Population Health Reporting to identify health gaps in care, and a financial analytical tool to support payer risk-based contracts and an integrated registry notification for appropriate care interventions.
Population Health Strategic Opportunities
The PHO engages in multiple opportunities across all payers to improve the quality of patient care, outcomes, and associated costs. Examples of opportunities include: deployment of a CAHPS Patient Satisfaction Survey, In-office Teleretinal Imaging, optimizing initiatives supporting TeleHealth engagement and performance, and Michigan Data Collaborative Type II Diabetes Initiative.
Population Health Onsite Assistance
Our team provides assistance with payer performance, quality data entry, work flows and infrastructure support, training, and on-going support for BCBSM PCMH designation.
About Us
With 215 sites of care across the state of Michigan, Ascension St. Mary’s Physician Hospital Organization is a joint venture between three Ascension affiliated hospitals; Ascension St. Mary’s of Saginaw, Ascension St. Mary’s of Standish, and Ascension St. Joseph Health Systems of Tawas and East Michigan Physician Organization (EMPO) which represents the physician clinical branch of the Physician Hospital Organization.
